To help protect your privacy and identity, we require patients to present a valid photo ID and supply a current email address in person and do not allow requests in any other way. This ensures patient records are loaded only to the authorized individual's FollowMyHealth account.

If you are currently scheduled for any upcoming service, you can enroll at that time. If you would like to begin enrollment in FollowMyHealth sooner, or have no pending services scheduled, please feel free to visit any Clinic facility and see your receptionist at your convenience to enroll. It just takes a few minutes to fill out the form, and we try to process these as quickly as possible.
