Living with Diabetes
Learning to manage diabetes is the best path to living well with diabetes. The Hutchinson Clinic provides people with diabetes the tools they need to manage their disease and lead a healthy lifestyle. Good blood sugar control will help people with diabetes get the most out of life and prevent or delay the onset of serious complications.
The Program
Our program will help you reach your diabetes management goals and provide you with a treatment plan designed specifically for you. The 10-hour course is delivered on two consecutive days and is available to children and adults with type 1, type 2 or gestational diabetes. Classes are held monthly. Evening classes are offered twice a year. Significant others are welcome to attend any or all of the sessions.
Diabetes Education Team
The Hutchinson Clinic has a multi-disciplinary team of diabetes experts who have received specialized education and training in state-of-the-art diabetes management. The team works together to develop a plan that will meet the needs of each participant. All areas are covered, including nutrition, exercise, stress management, and foot and eye care.
Our Diabetes Education Program is Accredited by DEAP.
Enrollment
Participants may enroll in the program by obtaining a referral from their primary care provider or by self-referral. Our diabetes team will work closely with your physician regarding your diabetes care plan. We encourage you to continue seeing your current primary care physician for all medical problems.
Setting Goals
Upon completion of the program, each participant will select at least one lifestyle goal that will contribute to improved health outcomes. The diabetes education team will assist you in selecting appropriate and attainable goals.
Follow-Up
An appointment will be scheduled with each participant approximately three months after they have completed the program to evaluate attainment of the goals selected and to assess blood sugar control. Additional follow-up appointments will be scheduled as needed.
Costs
Medicare and most insurance companies cover the cost of the diabetes program. Financial arrangements are made on an individual basis for anyone unable to pay for the program.
