Your FollowMyHealth® account is not the same as your electronic medical record (EMR). The electronic medical records has your full medical history dating back many years. Your FollowMyHealth® account will NOT contain your entire medical history.

If there is information that you are looking for that is NOT on your FollowMyHealth® account, you may request this information by signing a Authorization of Disclosure of Health Information and providing it to our Health Information Department; Monday – Friday 8 a.m. to 5 p.m.
